Antiinflammatoire mixte agissant dans l’inflammation primaire et secondaire au stade aig de l’inflammation: stabilise la membrane lysosomiale pendant la phase catabolique protolytique et augmente le tonus capillaire pendant la phase ractionnelle exsudative.
Agit au stade du granulome pendant la phase anabolique prolifrative de rparation: inhibition de la prolifration des fibroblastes, de la synthse des mucopolysaccharides, de la formation du collagne..

Voies d’administration

Posologie et mode d’administration
Dose usuelle par voie orale :
– chez l’adulte : 250 1000 microgrammes par kilo et par jour en 1 3 prises.
– chez l’enfant : 30 milligrammes par m2 de surface corporelle par jour en 1 3 prises.
– chez le nourrisson : 10 15 mg par kilo et par jour
en 1 3 prises, en traitement d’entretien de l’insuffisance surrnalienne ou des spasmes en flexion de l’enfant.
Dbuter toujours le traitement en augmentant progressivement les doses.
En cas de traitement de longue dure, la surveillance du traitement
doit porter chaque mois sur:
Tension artrielle, surveillance de l’tat digestif, ionogrammes sanguins et urinaires, bilan ophtalmologique, examen cardiovasculaire, radio pulmonaire.
L’arrt du traitement doit tre progressif: rduction de la dose
jusqu’ 7 8 mg (chez l’adulte) en 3 semaines, puis un mg tous les 15 jours pour un traitement de plus de 18 mois.L’insuffisant surrnalien doit porter une carte afin d’viter un arrt brutal de son traitement.
Utilisable par voie locale :
– en
applications cutanes.
* crme 1%: lsions relativement aigus, suintantes ou dermatoses des plis.
* pommades : lsions subaigus ou chroniques.
* lotions : lsions du cuir chevelu ou des oreilles.
– en pulvrisation nasale.
– en solution auriculaire.

Rsorption digestive rapide. La concentration plasmatique maximale est obtenue une heure aprs la prise.
Bonne rsorption aprs administration locale, sur la peau et sur les muqueuses
Rsorption cutane non ngligeable augmente par l’utilisation de
pansemants occlusifs, s’il existe une atrophie cutane ou si la couche pidermique est abrase, rode ou absente.
PROPORTIONNELLE A LA SURFACE CUTANEE RECOUVERTE ET AU RYTHME DES APPLICATIONS .
PLUS IMPORTANTE POUR LES POMMADES QUE POUR LES CREMES
Répartition
Diffusion rapide dans tous les tissus de l’organisme, en particulier dans le liquide cphalo-rachidien.
L’hydrocortisone ne franchit pas la barrire foeto-placentaire.
Concentrations plasmatiques relativement leves, principalement sous forme lie aux
protines (95%).
Demi-Vie
La demi-vie est d’environ 100 minutes.
Elle est allonge en cas d’insuffisance hpatocellulaire :

Métabolisme
Transformation hpatique pratiquement totale (99%) par des enzymes inductibles, en mtabolites inactifs hydrogns et conjugus (ttrahydrocortisone, ttrahydrocortisol).
Elimination
*Voie rnale : limination exclusivement par voie urinaire, principalement sous forme de glycuronides conjugus (99%) et en faible proportion sous sa forme initiale (1%).
*Voie biliaire : limination biliaire de 4% en 24 heures aprs injection
